Health Membership Program

We are excited to announce that we are moving to a more convenient and affordable health membership program.

We will only be accepting Blue Cross Blue Shield PPO and United Healthcare PPO insurances. This does include BCBS Anthem PPO and UMR (UHC) insurance plans. If you have an insurance we do not accept or you are self-pay and would still like to continue care with Mary as your provider, we have great options! Our membership allows you to still receive care from Mary at a lower rate. There is a one time per year $50 registration fee to get set up. We then have the easy option for auto-draft every month so your worry free on calling in to make a payment. If the membership is not for you, that is okay too! We will still be offering care to those who do not wish to sign up for the membership at a slightly higher rate. Due to the new model membership, we will not be offering walk-ins for the new year. We will be running by appointment only. You can reach us here on the portal, by phone or you can text our office number to reach a staff member! I have attached the cost breakdown for the membership/non-membership costs. We apologize for the delay with phone calls this sick season, we are working on improving and appreciate your patience with us!
